Community Living Specialist- Residential
Summary: The Community Living Specialist provides direct, face-to-face support to Individuals with Intellectual Disabilities (IDD) receiving waiver services in Community Living license facilities. This role involves adhering to all regulations, including Not Forgotten Policies and Procedures and 6100 regulations, and implementing established goals and outcomes assigned to individuals by the supports team. Responsibilities include assisting individuals with routine personal care tasks, adaptive daily living skills, physical transfers, and ambulation, while maintaining regulatory training requirements and documenting progress towards individual goals. The specialist also empowers individuals to participate in activities according to their plans, implements emergency procedures, and reports incidents per state and federal guidelines.
Responsibilities: The Community Living Specialist is responsible for informing the immediate supervisor of employment matters and care issues, instructing, and assisting with tasks to maintain a healthy and safe environment, and practicing universal safety precautions. They maintain professional relationships with families and person-centered team members, participate in support circles, and contribute to the development and revision of Individual Support Plans (ISP). Household tasks, nutritional meal preparation, and financial management support are also part of the role. Additionally, the specialist supports individuals' communication with families, community integration, and achievement of personal outcomes and goals. They are expected to perform additional duties as assigned, while demonstrating knowledge, skills, and abilities pertinent to the role, including effective communication, flexible scheduling, and familiarity with public and private resources and relevant laws.
Training and Experience: A minimum of a high school diploma and two years of experience working with IDD individuals are preferred. A valid driver’s license is required. Work will take place in the community and individuals' homes, and may involve transporting individuals in the specialist’s vehicle. Community Living Specialist - Habilitation
Summary: The Habilitation Specialist provides direct, face-to-face support to individuals with disabilities receiving waiver or based services from the provider. Collaborating with families, approvers, and contracted supervisors, the specialist creates, implements, and coordinates all aspects of Home and Community Services. Services are delivered in various settings, including a family's home, community activity sites, or the home of a certified caregiver. Responsibilities include adhering to established goals and outcomes as outlined in the ISP Plan, assisting individuals with personal care tasks, adaptive daily living skills, physical transfers, and ambulation. The specialist must complete 24 annual hours of training, document progress towards individual goals, and empower individuals to participate in activities according to their plans.
Responsibilities: The Habilitation Specialist is expected to implement emergency procedures, report incidents according to state and federal guidelines, and inform HR and immediate supervisors of relevant employment or care issues. They assist with tasks to maintain a healthy and safe environment, practice universal safety precautions, and maintain professional relationships with families and team members. The role includes participating in support circles, promoting self-direction in financial management, supporting communication with families, community integration, and the achievement of personal outcomes and goals. Additional duties may be assigned. The role requires clear and effective communication skills, flexible scheduling, and knowledge of available resources and services for IDD. A high school diploma is required, with two years of experience preferred. The work involves transporting individuals in personal vehicles and occurs in both community and home settings.
